PERFORMING ARTIST REGISTRATION FORM

This registration form was developed for the artist ( individual, group or choir ) that plans to sing during the awards celebration.  This registration form also includes the Welcome Service, Morning Service, Daily Power2Praise Sessions and Nightly Awards Programs.  It also includes admission to one of the luncheons,   1/4 page ad in the souvenir booklet and a table to promote your ministry on the day that you perform.  You get one luncheon (Media Luncheon or Founder's Luncheon) in your registration packet.  The second luncheon can be added for $45.
